Measurements Tab

The Measurements tab is an optional tab and is not enabled for all agencies/users. If you do not see it displayed in your client record, then your agency does not use it in their process.

The Measurements tool in OMC HMIS lets you track the level of vulnerability/stability for goal areas over time. The Matrix is also called a Self-Sufficiency Outcomes Matrix (SSOM). There is a standard SSOM sheet available to you. You can request access to the Measurements tab or request a customized list for your project by contacting your HMIS Lead.

  1. To record a measurement in the SSOM click on the Measurements tab, then click Add New Measurement.

  1. You may see other measurements that have already been recorded. If you are recording a new series, then click on Add New Measurement. If you are adding a new data point to an existing matrix series, click on Add New Point of Measurement.

  1. When you create a measurement, the first measurement will automatically be marked as the Initial Point of Measurement. Any measurements that occur between the initial and final measurements should be marked as Interim (see screenshot below). Any measurements that occurs after the final measurement should be marked as Follow Up (see screenshot below).

Notice that the measurement Provider automatically defaults to your ADMIN provider. That is okay since the Measurements tool is not connected to an Entry/Exit or Service Transaction. If this matrix is only for a specific provider in your agency, make sure to change the Provider from your ADMIN agency to the specific project.

  1. On the next screen, all of the questions in the measurement will be displayed:

  1. You only need to record rows for which you have a goal. For example, if the client has no children, then you can skip Parenting by clicking on N/A. You can fill in the responses on the screenshot above, or you can click the magnifying glass to see the full text of each question (screenshot below). If you want to fill out all the questions using this full text view, click Submit & Move to Next each time you complete a question.

  1. For subsequent Measurement points on a new measurement series, go back to the Measurement tab in the Client record and Add a New Point of Measurement to your existing Self-Sufficiency Matrix.

To see a report of your measurements, click on Show Summary button under the series you wish to view. The system can generate some summary reports for you.
