Initial Contact

Meet with/observe the client and collect as much information as you can.

  • Name and Current Living Situation can be collected as a baseline for record creation if the client is not willing to give further information at the time of initial contact.

  • Street outreach projects may create a client record with limited information about the client and improve on the accuracy and completeness of client data over time by editing data in HMIS as the client relationship evolves.

  • Once the client is engaged, the expectation is that all required data elements are collected and entered into HMIS. Collected information must be checked/confirmed during engagement.

Naming Convention – A Code Name can be used to enter and track observed clients into HMIS if needed, E.g., Redhat Tenthstreetbridge. The code name will eventually need to be updated in HMIS. Please remember to do so.
