Case Plans Tab
Using the Case Plans tab is optional but can be useful for case management. This is also where you can store sensitive information in OMC HMIS because case notes are not shared with other providers.
File attachments can be uploaded at any time by clicking Add New File Attachment, but all other case notes must be attached to a goal. Click Add Goal to create a new goal, or the pencil icon to edit an existing goal.
The Print Case Plan button just generates a PDF of the goals associated to the client.

When you have added or edited a goal, a new window will appear that has several different sections you can fill out, including a description of the goal, the status of the goal, and case notes. Your agency may have specific requirements for which parts of the goal need to be filled out.
You can also schedule a follow up reminder for a future date. Enter the projected follow-up date and select what HMIS user should get the follow up notification. This notification will appear on that user’s HMIS homepage.
Clicking the Goal hyperlink will take you back to the goal page so you can update the outcome. Once you input the Completed Follow Up Date, this goal will disappear from your Follow Up list.
